Welcome to the Ontario Children/Youth Camp application!

Before filling out this application, you can learn all about the camp and its class and choir
offerings for different age levels by visiting our Children/Youth Camp web page.

1. This form is for children and youth who are 8-17 years old by the first day of camp.

2. If you are 16-17 years old and are participating in the Youth Camp, but also wish to
participate in an evening adult choir, please complete this application, then contact the
registrar to add an evening choir (or two). If you are 16-17 years old and wish to participate
in the Adult Camp, please use the Adult Camp application instead of this one.

3. Your application cannot be saved partway through and returned to later; it must be ﬁlled
out in one sitting. If you close or refresh the page, information you have entered will be lost.

4. If you wish to pay by credit card, non-refundable fees of approximately 5% will be added to
your amount owing. These fees are collected by the application platform and do not go to
the camp. To avoid these fees, you may choose to pay by Interac e-transfer or cheque
instead of credit card.

5. All prices are in Canadian dollars (CAD). If your credit card account is a different currency,
exchange rates and fees are determined by your credit card provider. You may need to call
your bank or credit card company to authorize the transaction. If the transaction is
declined, call your bank prior to leaving the application page. You will lose any data you
entered if you leave the page.

6. Some applicants have had difficulty using certain web browsers and platforms with this
application. If you are having any trouble that appears to be a browser issue, consider
using a different browser (Chrome, Firefox, etc.). If you are completing this application on a
phone or other hand-held device and it is not working, consider switching to a computer
browser.

Child Safety

I understand that SCMC instructors and staff will provide supervision for the safety and well-being of my child/youth at camp. I further understand that I am responsible to drop off and pick up students at scheduled times.

Attendance

I understand that my child/youth is required to attend all scheduled children's camp activities including choir rehearsals, class, Dress Rehearsal, and ChoralFest.

Terms & Agreements

Since a major part of music camp is participation in the Choir Sing at the conclusion of camp, we require all children to be committed for the camp week and the weekend activities, including both Sunday programs.

All fees are refundable MINUS the Application Fee and processing fees through July 15. No refund will be given after July 15.
